Nursing Home Grants

Eligible Nursing Homes will receive:

  • A complete copy of the VDT program including components
  • Virtual Dementia Tour conducted by a Second Wind Dreams certified trainer
  • Person-centered assessment of staff performance
  • VDT facilitator training for two designated staff
  • CEU credit for multiple domains
  • Staff in-service training

    Dear Second Wind Dreams Grant Team,

    is pleased to support Second Wind Dreams in their application for funding to bring the Virtual Dementia Tour® (VDT®) to our state, ,through the CMS Civil Money Penalty Fund grant program . The VDT® is an evidence-based, patented educational tool that assists people in identifying with and better understanding the needs of those living with dementia which results in an improved relationship between caregiver and resident. We support the mission of Second Wind Dreams to provide essential staff training on person-centered dementia care while improving the quality of life for nursing home residents in our nursing homes.
